Dr. Ratched is one of eight looks. Any type can use a model unless a type rule forces male or female. The look does not set evidence.

Look, skins, and the limit
White coat, blue scrubs, gloves, gray skin, deep eye sockets, and a small head joined at the mouth. V1.3 changed the mesh heavily, so older thumbnails are not the current body. Skins: Human Doctor from the t-shirt, Plague Doctor from a plush preorder, and Fairy King from the 2026 pixie pack in V1.12. The two-headed read is the sculpt, not a mechanic. Any type can wear the current mesh, including types that are not tied to a gender.
What stays on the type pages
Siren and Keres are always female models. Dullahan is always male, and photos of that type have no head. Those rules live on the type pages. This page does not link a model to one type.
